Forms for Submitting Administrative Rules
APA filing forms are provided by the Office of the Secretary of State for the express purpose of filing administrative rules governed by Title 3 V.S.A. Chapter 25.
The forms are available for download in Microsoft Word format. The forms are designed to be user friendly, and users may tab through the fields typing directly into the form. Please note that some fields are locked, the limitations on characters lengths for these fields are preset with the maximum character lengths allowed in the current APA Tracking Database.
While you should still proof your forms carefully, new forms are available that include an embedded macro to activate the spell check utility included in Microsoft Word. If the spell check button does not work properly check the macro security settings for WORD. If the setting is too high the application (WORD) has probably disabled the macro. Alter the settings to enable the macro or the feature will not work. If you have questions regarding this macro please contact .
Each rule filing requires specific forms (i.e. each filing has its own Coversheet). Please choose the forms appropriate to your filing needs. Click the links below for the corresponding form.
The latest revision to the forms will occur July 31, 2010. If the notation in the footer of the form does not reflect this date please refresh the page and re-download. If the form is still not correct please contact our office and we will remedy the situation.

It is recommended that users prepare filings on freshly downloaded forms rather than storing forms for future use, to avoid use of outdated forms. The Office of the Secretary of State may be unable to accept filings made on outdated forms.
